You service your car. You service your boiler before winter. You probably even have someone come round to check the gutters.

Your mouth? Most people wait until something hurts.

This approach makes a certain kind of sense. Your car makes worrying noises before it breaks down. Your boiler shows warning signs. But your teeth? They can be quietly deteriorating for months before you feel anything.

By the time you notice, what could have been a £80 filling has become a £800 root canal. Or the gum disease that would have responded to a simple deep clean now requires surgical intervention. Or the slightly loose tooth that “seemed fine” has reached the point where extraction is the only option.

Spring is when we service everything else. Perhaps it’s time to apply the same logic to the thing that directly affects your ability to eat, speak, and feel confident in social situations.

The Economics of Dental Neglect

Let’s be specific about the cost progression, because vague warnings don’t motivate anyone.

A small cavity caught early: one filling, one appointment, typically £80-150 depending on the tooth and material.

That same cavity left for a year becomes a larger cavity. Now it’s compromised more of the tooth structure. You need a more extensive filling or possibly an onlay. Cost: £200-400.

Leave it another year. The decay reaches the nerve. Now you’re in pain, and you need root canal treatment. That’s multiple appointments, specialist work, and typically £600-900 for a back tooth.

But root canal treatment weakens the tooth structure, so you also need a crown to protect it. Add another £800-1,200.

Total cost of ignoring that original cavity for two years: £1,400-2,100. Plus the time off work for multiple appointments. Plus the pain of waiting until it became an emergency.

Now multiply that by the fact that most people don’t have just one problem developing. They have several in different stages.

This isn’t scare tactics. It’s the actual progression timeline that plays out constantly.

What a Spring Check Actually Catches

The value of a routine check-up isn’t confirming everything is perfect. It’s catching problems when they’re still minor.

Early decay: Cavities start small. At the initial stage, they’re not even cavities yet—they’re areas of demineralisation that can sometimes be remineralised with fluoride and improved cleaning. Once decay breaks through the enamel surface, you need a filling. But catching it at the “watch and prevent” stage costs you nothing beyond the check-up.

Gum disease progression: This is the silent problem. Early gum disease (gingivitis) causes bleeding when you brush. It’s reversible with proper cleaning and a hygiene appointment. Once it progresses to periodontitis, you’re dealing with bone loss that’s permanent. The gum tissue recedes, teeth become loose, and eventually you’re looking at extractions.

Dr. Shivani Patel, who holds a Master’s degree with distinction in Periodontology from UCL, sees this progression regularly. Her approach involves screening for early signs—pocket depths, bleeding points, areas of recession—that most patients wouldn’t notice themselves. By the time you notice, it’s typically past the “easily reversible” stage.

Failing old work: Fillings don’t last forever. Crowns can develop decay at their margins. Bridges can lose their seal. A check-up spots these issues before the old restoration fails completely and causes more damage to the underlying tooth.

Oral cancer screening: Mouth cancer is one of the fastest-growing cancers in the UK. Early detection is crucial. Every routine examination includes checking the soft tissues of your mouth—tongue, cheeks, palate, throat. Most suspicious lesions turn out to be nothing, but the ones that aren’t nothing need to be caught early.

The Hygienist as the Early Warning System

There’s a tendency to view hygiene appointments as optional extras. Nice to have, but not essential.

This fundamentally misunderstands what a good hygienist does. They’re not just cleaning your teeth—they’re providing the preventative care that keeps you out of the treatment chair.

She removes the calculus (hardened plaque) that your toothbrush can’t shift. She accesses areas you can’t reach properly at home. She spots early gum problems before they become serious. And crucially, she educates you on what’s actually happening in your mouth and what you can do about it.

Most people who avoid hygiene appointments do so because they find them uncomfortable. The sensitivity, the scraping sensation, the bleeding. Here’s what that discomfort is telling you: your gums are already inflamed. Healthy gums don’t bleed during cleaning. Avoiding the hygienist doesn’t make the problem go away—it makes it worse.

The payoff for regular hygiene care is less invasive treatment over time, not more. Your gums become healthier. Cleanings become easier. You retain your teeth longer.

The Whole-Family Approach

Spring check-ups work particularly well for families because you can coordinate everyone’s appointments in the same window. One morning, entire family sorted.

But children’s check-ups serve a different purpose to adult ones. You’re not just checking for problems—you’re establishing good habits and catching developmental issues early.

For children: We’re looking at tooth eruption patterns, jaw development, early signs of crowding. We’re applying fluoride to strengthen developing teeth. We’re providing fissure sealants to protect vulnerable back teeth. And we’re normalising the experience of visiting the dentist so they don’t develop anxiety later.

For teenagers: This is often when orthodontic needs become clear. Permanent teeth are in, jaw growth is nearly complete. If braces or aligners are going to be needed, this is the optimal window. We’re also watching for wisdom teeth development and checking for cavities in newly erupted molars.

For adults: We’re monitoring existing work, screening for gum disease, checking for grinding damage (especially if you’re stressed), and looking for early signs of more serious issues.

For older adults: Gum recession, root decay, and dry mouth become more common. Medications can affect oral health. Existing dental work may need repair or replacement. The focus shifts to maintenance and preservation.

Different life stages, different priorities. But all caught in the same proactive appointment window.

When Prevention Has Already Failed

The other reason spring works for dental checks is that it catches the people who’ve been putting off dealing with something.

You’ve had a twinge. Or sensitivity. Or you know that filling is loose. You’ve been meaning to call for three months, but it hasn’t become urgent enough yet.

Then it does. Usually at the worst possible time—right before a holiday, during a busy work period, or on a Friday evening when you can’t get an appointment.

Some people avoid routine check-ups because they don’t have dental insurance and assume they can’t afford it.

Let’s do the maths on this, because the calculation isn’t obvious.

A routine check-up costs approximately £60-80. A hygiene appointment costs £60-100 depending on what’s needed. If you’re doing this twice a year, you’re spending £240-360 annually on preventative care.

Now compare that to the cost of neglect: one root canal and crown (£1,400-2,100), or one implant to replace a lost tooth (£2,000-3,000), or periodontal treatment for advanced gum disease (£500-2,000).

Prevention is always cheaper than treatment. The question isn’t whether you can afford check-ups. It’s whether you can afford not to have them.

Spring works psychologically for the same reason January does: it’s a natural reset point. Winter is over. You’re thinking about renewal and maintenance anyway.

But spring has an advantage January doesn’t: you’re not competing with every other resolution you’ve made. The gym motivation has faded. The diet has been adjusted to something sustainable. You have mental bandwidth available.

And practically, you’re not waiting until summer when you might be on holiday, or autumn when things get busy again, or winter when problems that could have been caught early have had another eight months to worsen.

The best time to service something is before it breaks down. The second-best time is right now, before the breakdown costs you significantly more.

What Happens at the Actual Appointment

If you haven’t had a check-up in a while, here’s what the appointment actually involves, because uncertainty creates avoidance.

The dentist examines each tooth, checking for decay, cracks, or wear. They check your existing fillings, crowns, or other work. They examine your gums, measuring pocket depths if there are concerns. They screen the soft tissues of your mouth. They may take X-rays if it’s been more than a year or if there’s a specific concern.

Then they tell you what they’ve found. If everything is fine, you’re done. If there are issues, they explain what needs addressing and in what priority.

You’re not forced into immediate treatment. You’re given information. You can ask questions. You can book treatment for a later date. You can get a written treatment plan to consider.

The appointment takes 20-30 minutes. You leave with either peace of mind or a clear plan. Both are valuable.

The Spring Clean for Your Mouth

You’re going to deep-clean your house this month. You’re going to sort through the garden. You’re going to deal with all the small maintenance tasks that accumulated over winter.

Your mouth deserves the same attention. Not because teeth are like boilers, but because they’re more important. You need them every single day, multiple times a day, for eating, speaking, and feeling confident.

The £5,000 in the headline isn’t hyperbole. It’s the realistic cost of ignoring small problems until they become large ones. It’s what happens when prevention is skipped in favour of crisis management.

You wouldn’t skip servicing your car and then act surprised when the engine fails. Don’t do the same with your teeth.
